Those wishing to join a BTM Referral Group must complete application below. If you wish to submit your application for consideration in one of these future groups, please select the appropriate location in the field below.  Please complete your application thoroughly and thoughtfully.  The Membership Committee for the Referral Group will review your application, notifying you of their decision.

NOTE: Submitting an Application for a BTM Referral Group does not guarantee Membership. Applications received will be reviewed in their entirety.
